AbbeHealth Services - is a regional nonprofit organization dedicated to providing mental health and aging services that help individuals achieve their optimum levels of independence and success.
The Direct Support Professional (DSP) Day Program position is responsible for the provision of Home-Based Habilitation skill-based training, support, and/or treatment services to individuals in recovery from a serious mental illness in a group setting. Services are provided in at the Day Program location or community locations. Day Program services are designed to foster the acquisition of skills, positive social behavior, increased independence, personal choice, community participation, and development of social roles and relationships.

Responsibilities
Facilitates groups and provides skill-based training, support, and interventions to clients in a group setting at the Day Program location or in the community. Services provided are unique to the individual as identified in their person-centered service plan goals. Services provided are unique to the individual as identified in their person-centered service plan goals and may include but are not limited to community participation, development of hobbies and leisure activities, and self-advocacy and self-determination. Participates in client service planning and development of measurable goals based on client needs and desires. Recognizes symptoms of physical and mental illness and performs appropriate interventions, communication, and/or referrals in accordance with the identified crisis plan. Consults with natural and professional supports and community agencies to assist clients with living in the least restrictive setting possible. Advocates for client rights and choice in obtaining needed services. Attends regularly scheduled staff meetings, rounding, and in-services. Participates in and completes all required training by identified due date. Participates in the planning, implementation, and evaluation of programming effectiveness. Completes documentation of services provided accurately, thoroughly, and timely within established guidelines. Ensures documentation meets billing requirements and completes necessary corrections timely as required.
Qualifications
Education
High School diploma or equivalency required.
Associate’s degree in human service-related field preferred.
Qualifications/Experience
No experience necessary, 1 year of mental health and/or other disability experience preferred.
Valid Drivers License Required
