Location: Newton, KS

Schedule: Monday – Friday (Standard Day Shift)

Pay: $24.15 per hour + monthly bonus opportunities

Are you looking to make a direct, life-changing impact outside the walls of a traditional clinic? Prairie View is hiring an ACT Case Manager to join our dedicated Assertive Community Treatment (ACT) department!

Our ACT department is at the forefront of non-traditional, community-based care. As a crucial member of a multidisciplinary team, you will deliver intensive, hands-on support directly where clients need it most—in their homes and out in the community. ACT works with adults experiencing serious and persistent mental illness, co-occurring substance use, and complex physical health conditions. Your mission will be to reduce emergency room visits, prevent inpatient hospitalizations, and empower individuals to live independent, fulfilling lives.

Key Responsibilities

Direct Clinical & Community Work

  • Provide strengths-based case management, assertive outreach, and crisis intervention in the community.

  • Support clients in developing essential daily living skills (e.g., food preparation, housekeeping, personal hygiene, and budgeting).

  • Engage clients in health literacy, wellness education, and self-management of physical and mental health conditions.

  • Help clients set personal recovery goals, navigate community resources, and build natural support networks.

  • Assist in leading psychosocial groups and advocating for client rights across social and healthcare systems.

Administrative & Team Collaboration

  • Participate actively in daily ACT multidisciplinary team huddles, clinical supervision, and agency meetings.

  • Collaborate closely with internal clinicians, external agencies, and community partners.

  • Maintain organized daily schedules and complete thorough, timely clinical documentation (progress notes completed within 72 hours).

  • Complete state-required reporting (e.g., AIMS) and comply with all Prairie View quality standards.

Work Environment & Logistics

  • Field-Based Care: The vast majority of services take place directly in participants' homes and community settings.
