WHAT IS THIS POSITION RESPONSIBLE FOR?

  • Evaluate and screen applicants appropriateness for admission to the program.
  • Coordinate admissions to the service, as well as discharges from the program
  • Monitor living environment, involvement in community living and self-help groups, employment, and payment of fees.
  • Maintain adequate inventory of food and household supplies.
  • Monitor utilization of services and adhere to quality care standards.
  • Monitor residents compliance with transitional residential services guidelines.
  • Coordinate reentry into the community.
  • Provide skills training and care coordination to Transitional Residential patients.
  • Develop appropriate treatment plans and goals and monitor patient’s progress toward treatment goals.
  • Coordinate patient care with other members of the treatment team.
  • Maintain contact with referral sources and other providers as necessary.
  • Maintain appropriate clinical documentation.
  • Provide care coordination to clients in other addiction programming (residential and outpatient) as needed.
  • Collaborate with nursing staff and collaborating physicians to ensure high quality of medical care for clients.
  • Develop and sustain relationships with other providers in the community to provide for on-going client needs.
