Admissions Intake Representative, Licensed Practical Nurse (LPN), Full-time

Job Summary

The Clinical Intake Representative is responsible for managing all intake processes required to efficiently process referrals for admission to Hospice Savannah, Inc. services. This includes reviewing all clinical records for eligibility, reviewing payer sources, obtaining all pre-certifications, managing incoming calls, scheduling visits and education patients and families on services within Hospice Savannah, Inc. when appropriate. The Clinical Intake Representative is committed to the philosophy, goals and objectives of the Hospice Savannah concept while consistently demonstrating the Hospice Savannah values and compliance with organization’s services excellence standards.
