GENERAL RESPONSIBILITIES:
- Provide Case Management (CPST, Crisis) to persons assigned to caseload.
- Provide goal-directed support and solution-focused interventions.
- Work hours are flexible to meet the needs of the person served.
- The majority of contacts happen out of the office and in community settings.
- Contact with new referral is made within 48 hours of assignment.
ESSENTIAL JOB FUNCTIONS:
- Will make all contacts as indicated in the tx plan, and if going to be out of the office, makes arrangements with other staff to ensure contact continues.
- Completes no-show/cancelation log as required in individual program.
- Weekly contact sheets are turned in at the end of your work week.
- Progress notes are completed within 24 hours of service.
- Makes appropriate referrals to PS/PRI/SE and completes required paperwork.
- Completes assessments within the required timeframe.
- Will make regular contacts with the tx team
- EMR scheduler is utilized and kept updated.
