Shift Details: FT - 40 hours/week

Schedule: Monday-Friday from 8:30am-4:30pm

Compensation: $53,500-$57,000 annual salary

Location: Hybrid - Vona Center for Mental Health: 9220 Bass Lake Rd #255, New Hope, MN 55428

About the Job:

Maintain a caseload of adult clients who have a serious and persistent mental illness (SPMI) and provide services in the four core case management components-assessment, service plan development, referral and linkage to services, and coordinating and monitoring service delivery.

Essentials:

Direct Client Services

• Provide face-to-face and collateral contacts with clients as required by Minnesota TCM standards and individualized service plans, prioritizing in-person services.
• Deliver services that support client access to needed medical, social, educational, vocational, housing, and other mental health services.
• Assist clients in developing and maintaining skills necessary for community living and stability consistent with rehabilitative and recovery-focused principles.
• Coordinate with emergency and crises services in accordance with agency protocol and mandatory reporting laws.
• Travel to service settings, including client homes and community locations, as required for service delivery.

Assessment & Service Planning

• Complete and/or update the Functional Assessment (FA) at intake and at least every 180 days, in accordance to MN DHS standards.
• Develop, implement, and maintain the Individual Community Support Plan (ICSP) that is:
  • Person-centered and culturally responsive
  • Based on the Diagnostic Assessment and Functional Assessment
  • Inclusive of measurable goals and objectives
• Monitor and document client progress towards goals and modify plans to reflect changing needs.

Care Coordination & Collaboration

• Coordinate services with the client, family, and multidisciplinary team members in compliance with TCM service requirements.
• Facilitate referrals and linkage to community resources, including but not limited to healthcare providers, schools, housing agencies, and social services.
• Participate in routine clinical supervision, case consultations, team meetings, and interagency planning to support integrated service delivery.

Documentation & Regulatory Compliance

• Complete timely, accurate, and compliant documentation of all services in accordance with MN DHS Rule 79 requirements and agency policy.
• Maintain complete and up-to-date client records, including:
  • Functional Assessments
  • ICSPs
  • Meeting and collateral notes
  • Required authorizations and legal documentation
• Maintain strict adherence to HIPAA and MN data privacy laws (Minnesota Government Data Practices act, where applicable)

Hennepin County AMH-TCM Responsibilities

• Act as the designated agency representative under MN Statutes Chapter 253B for clients subject to civil commitment, including commitment orders, stays of commitment, provisional discharges, and commitment settlement agreements.
• Monitor and fulfill designated agency responsibilities related to court-ordered mental health commitments in accordance with county, state, and agency requirements.
• Prepare, complete, and submit all required documentation related to civil commitment proceedings within established timelines and regulatory standards.
• Coordinate with clients, treatment providers, attorneys, court personnel, and other stakeholders to support compliance with civil commitment requirements.
• Attend and participate in commitment court hearings, review hearings, and other legal proceedings as requested or required.
• Maintain accurate, timely, and compliant records related to civil commitment activities and court involvement in accordance with MN DHS requirements, court mandates, and agency policy.
• All other duties as assigned.

Required Qualifications:

• Bachelor’s degree in behavioral sciences or related fields or qualify as a Mental Health Practitioner under section 245I.04, subdivision 4.
• Have experience and training in identifying and assessing a wide range of children and/or adult needs.
• Be knowledgeable about local community resources and how to use those resources.
• 2000 hours of supervised experience in the delivery of mental health services to children and/or adults (depending on role), preferred.
