Daily Responsibilities:

  • Conduct telephonic prospective, concurrent and retrospective review of active patient care.
  • Coordinates treatment plans and conduct transitional care calls.
  • Prioritizes patient care needs.
  • Developed care plans with time frames and obtainable outcomes.
  • Reevaluate care plans, time frames and outcomes.


Requirements as Ambulatory Case Manager:

  • Bachelor’s degree in Nursing
  • Active California RN license in good standing
  • 3+ years of Clinical Nursing with 1 of those years being within an Outpatient setting

Pay Rate: $38/hr. - $41/hr.

Hours for this Position:

  • Shift: Monday-Friday 8-5

Ambulatory Care Nurses take care of patients in environments outside of hospitals. Their primary focus lies in pain management and general health education for patients with chronic injuries or illness. They provide the medical screenings, triage, and case management that help restore a patient’s ability to live independently.
