Daily Responsibilities:
- Conduct telephonic prospective, concurrent and retrospective review of active patient care.
- Coordinates treatment plans and conduct transitional care calls.
- Prioritizes patient care needs.
- Developed care plans with time frames and obtainable outcomes.
- Reevaluate care plans, time frames and outcomes.
Requirements as Ambulatory Case Manager:
- Bachelor’s degree in Nursing
- Active California RN license in good standing
- 3+ years of Clinical Nursing with 1 of those years being within an Outpatient setting
Pay Rate: $38/hr. - $41/hr.
Hours for this Position:
- Shift: Monday-Friday 8-5

Ambulatory Care Nurses take care of patients in environments outside of hospitals.
