Minimum Qualifications: PharmD from an ACPE-accredited program; PGY2 ambulatory care, HSPAL, or equivalent experience; eligible for Indiana licensure; demonstrated leadership experience. Preferred Qualifications: Three years of supervisory experience; strong communication, process improvement, and team leadership skills.
