This is a year-long, full-time AmeriCorps position requiring completion of a minimum of 1,700 service hours. The work schedule for this position is aligned with Erie's hours of operations.
Main Duties & Responsibilities:

Provide one-on-one health education and self-management support to patients

Participate in community events, training, and AmeriCorps service activities

Support Lending Hands for Life HIV programming and re-engagement efforts

Conduct outreach and engagement for patients requiring additional support

Document patient interactions and coordinate follow-up care activities

Facilitate health education groups and recruit participants for programs
