Join AmeriCorps as a Health Educator with a focus in Health Promotions at Erie Family Health Centers. Members provide one-on-one health education, facilitate group sessions, conduct patient outreach and disease management assessments, and help patients develop healthy lifestyle habits through in-person and telehealth services. This is a year-long, full-time AmeriCorps position requiring completion of a minimum of 1,700 service hours. The work schedule for this position is aligned with Erie's hours of operations.
Main Duties & Responsibilities
- Provide individualized health education and lifestyle coaching to patients, helping them manage chronic conditions and improve overall health outcomes through in-person and telehealth visits.
- Assess patient needs, identify barriers to care, and connect patients with appropriate programs, resources, and support services.
- Facilitate health education groups and disease prevention programs while recruiting and engaging patients to participate in available services.
- Conduct patient outreach, follow-up, appointment scheduling, and care gap re-engagement activities to support continuity of care.
- Collaborate with healthcare providers, care teams, and community partners to coordinate services, support program goals, and improve patient outcomes.
- Participate in community engagement activities, health fairs, educational events, and quality improvement initiatives that promote health and wellness in underserved communities.
