Join AmeriCorps as a Social Drivers of Health (SDOH) Health Educator at Erie Family Health Centers. This year-long, full-time service position provides the opportunity to help connect individuals and families to resources that support their health and well-being. This is a hands-on experience in community outreach, resource navigation, benefits enrollment, and patient support while building professional skills through training and service.

This is a year-long, full-time AmeriCorps position requiring completion of a minimum of 1,700 service hours. The work schedule for this position is aligned with Erie's hours of operations.
Main Duties & Responsibilities:
- Screen individuals and families for social drivers of health needs and connect them with appropriate resources.
- Provide referrals and linkage to community services including food, transportation, financial assistance, and other support programs.
- Assist with enrollment in eligible benefits, programs, and services.
- Support outreach activities and follow-up with individuals regarding resource needs.
- Provide digital navigation assistance and help individuals access available services.
- Complete documentation, data entry, and program tracking activities.
