AOT/ACT Therapist

Summary

Provide clinical AOT/ACT therapy for adults with serious mental illness, coordinate care plans, and ensure regulatory compliance while collaborating with treatment teams.

Job Description: Lead Assisted Outpatient Treatment (AOT) and Assertive Community Treatment (ACT) programs and provide therapeutic AOT and ACT services for adult participants with SMI, per the participants’ person-centered recovery plans (PCRPs) to improve treatment adherence and reduce the consequences of non-treatment.

Role and Responsibilities

  • Provide direct clinical services for participants assigned to caseload, through the helping process of engagement, assessment, planning, intervention, evaluation, and termination, as indicated by the treatment team and PCRP.
  • Serve as case coordinator for participants assigned to caseload.
  • Maintain understanding of and ensure compliance of all regulatory standards for AOT, ACT, In-Reach, IHope or assigned CSS services.
  • Focus on growth, innovation, and program advancement in consultation with the VP CSS and agency vision.
  • Recruit referrals and generate referrals to other programs/resources to promote community reinforcement for participants.
  • Clinical duties include providing individual therapy, group therapy or crisis services, maintaining EMRs and completing treatment documentation as required.
  • Administrative duties include supervision of direct reports, participating in community outreach for CSS services, collaborating with community partners, completing growth goals, data collection, evaluation, and state reports.
  • Collaborate with multiple disciplinary treatment teams and maintain collateral contacts with significant others as needed to foster participants’ continuity of care.
  • Maintain all requirements for licensure or eligibility per the state licensing board including participation and documentation of clinical supervision.
  • Meet agency clinical productivity requirements.
  • Serve in after hour on call rotation.
  • Other duties as assigned.

Qualifications and Education Requirements

Must be able to successfully pass background checks and drug screen.

  • Master’s degree in psychology, social work or a human service-related field.
  • Licensed or license eligible by corresponding Kentucky State Board.
  • Clinical experience preferred.
  • Experience with SMI population preferred.

  • Travel may be required.
  • Valid driver’s license.
  • Dependable transportation.
