WHAT YOU WILL BE DOING:
- Providing comprehensive health care to residents in:
- The Health Care Clinic
- Skilled Nursing facility
- Assisted Living facility
- Private Homes
- Working with geriatric patients to develop and implement a comprehensive plan of care including:
- Assessment
- Diagnosis
- Treatment
- Management
- Education
- Health Promotion
- Care Coordination
- Collaborating and coordinating with other health team members to achieve desired patient outcomes.
- Serving as a leader and role model for professional development of peers and others.
WHAT WE ARE LOOKING FOR:
- Graduate from a Nurse Practitioner or Physician Assistant program.
- Valid Nurse Practitioner or Physician Assistant License.
- Valid DEA license.
- Valid Driver’s License.
- Previous Gerontology experience.
- Experience in Urgent Care, Primary Care, Assisted Living and Skilled Nursing facilities preferred.
- Effective verbal and written communication skills.
- Interpersonal skills necessary to work productively with patients, families, nursing staff, and other departments.
