Assessment Clinician/Therapist - New Mexico ALB

The Assessment Clinician is responsible for conducting comprehensive, person-centered biopsychosocial assessments to determine diagnostic impressions, identify clinical needs and risks, recommend the appropriate level of care, and facilitate timely access to behavioral health services. This position serves as the clinical entry point into services, ensuring assessments are clinically sound, strengths-based, and aligned with agency, state, and CCBHC standards.

Essential Functions:

The Assessment Clinician will:

  • Conduct comprehensive biopsychosocial assessments utilizing person-centered, trauma-informed, and culturally responsive practices.
  • Complete diagnostic evaluations utilizing DSM-5-TR criteria and clinical formulation.
  • Assess suicide risk, homicide risk, substance use, trauma history, medical needs, and social determinants of health.
  • Interpret required screening and measurement-based care tools.

  • Determine the appropriate level of care and make clinical recommendations based on assessment findings.
  • Collaborate with clients to identify treatment goals and immediate service needs.
  • Coordinate referrals to appropriate internal and external programs.
  • Ensure clients requiring urgent intervention are connected to crisis services when indicated.
  • Complete all assessment documentation accurately and within required timeframes.
  • Communicate assessment findings to the assigned treatment team to support continuity of care.
  • Participate in quality improvement initiatives and clinical consultation related to assessment practices.
