Assessment Coordinator
Job Summary: The Assessment Coordinator is responsible for conducting level of care assessments which involve interviewing individuals, documenting assessments, handling funding requests, and managing case services related to patient admissions into treatment.
Responsibilities and Duties:
- Completes level of care assessments for individuals seeking substance abuse treatment using ASAM criteria.
- Completes precertification with insurance company or county funding source to facilitate admission to the appropriate level of care.
- Provides case consultation and documentation to contractual agencies.
- Documents case management and clinical assessment in the EMR and Pennsylvania Web Infrastructure for Treatment Services (PA WITS) system.
- Ensures clinical record is compliant with all licensing, regulatory and accrediting agency requirements.
- Encourages individuals to understand the benefits of recommended level of care.
- Collaborates with other departments to ensure prompt admission to treatment.
- Completes urine drug screens as needed for outpatient assessments.
- Provides clerical support when needed to assist admission staff with duties such as answering phones, completing intakes and verifying insurance.
- Participates in supervision, department meetings and trainings as required.
- Maintains a flexible daily schedule and adapts as needed.
- Performs other related duties as assigned.
