Assisted Living Clinical Coordinator RN

Job Requirements:

    • Licensed as a registered nurse in the State of Oklahoma.
    • Minimum of two (2) years work experience as a nurse in a long-term care environment preferred.
    • Current BLS/CPR certification or ability to obtain within the required timeframe.
    • Meets all state requirements for and is certified in Food Handling.
    • Ability to assess residents, recognize changes in condition, prioritize clinical needs, and respond appropriately in an assisted living environment.
    • Ability to maintain accurate and timely clinical documentation.
    • Ability to communicate effectively with residents, families, physicians, healthcare providers, and staff.
    • Commitment to person-centered and resident-directed care is essential.
    • Must have knowledge of federal and state regulations governing clinical care and nursing practice in long-term care.
    • Must have demonstrated leadership qualities, supervisory skills, and interpersonal relationship skills.

    Responsibilities:

    Supports the clinical and operational needs of residents and Assisted Living by providing nursing oversight, clinical guidance, and direct assistance within the scope of licensure, training, and facility policy.

    • Daily
      1. Review nutritional reports and collaborate with dietician, director, and physicians to accommodate resident diets while maintaining resident preferences.
      2. Review pharmacy reports.

      3. Participate in QI meetings.

    • Quarterly
      1. Review nutritional reports and collaborate with dietician, podiatrist, audiologist, director and physicians to accommodate resident diets while maintaining resident preferences.
      2. Review pharmacy reports.
      3. Participate in QI meetings.
      4. Review and Complete Pain Assessments
      5. Review Labs Results

      • Monthly
        1. Assist in nurse education as needed.
        2. Complete resident assessments and reassessments, including annual assessments and assessments prompted by significant changes in condition.
        3. Review medication orders to ensure accuracy and assist in education as needed.
        4. Review infection control with director.
        5. Assist in reviewing and updating resident care plans with director.
        6. Complete required monthly medication reviews and send to physicians.

      • Weekly
        1. Track and complete destruction of medications as needed.
        2. Review incident reports with director and follow up on clinical concerns as needed.
      • PRN
        1. Complete move in assessments within 30 days prior of move-in or on day of move-in.
        2. Review move-ins to ensure all clinical data was collected.
        3. Reassess pain assessments that are greater than a 5.
        4. Assess residents experiencing a significant change in condition and coordinate follow-up as needed.
        5. Reassess appropriateness for Assisted Living
        6. Communicate significant changes in condition to physicians/providers and follow up on orders as needed.
