Coordinate eldercare services for seniors by assessing needs, recommending care plans, and enrolling clients into appropriate programs while collaborating with internal and external partners.

Join our Integrated Community Care Provider (ICCP) team and make a meaningful impact in the lives of seniors and their families. In this role, you will work closely with internal and external partners to understand the needs of seniors and recommend the most appropriate services. Comprehensive on-the-job training will equip you with the knowledge and tools to thrive and grow in your role.
You will need to:
- Develop a strong understanding of eldercare services to effectively meet the needs of seniors.
- Collaborate with internal teams and external partners including organising meetings and tracking follow-ups closely to ensure seamless support for seniors and families.
- Manage clients' cases, assess customers' needs, recommend suitable services and facilitate their enrolment to services.
- Prepare workflows, presentations and other supporting materials.
- Generate reports for submission to regulators and stakeholders.
You will need to:
- Develop a strong understanding of eldercare services to effectively meet the needs of seniors.
