Atlantic Health Asst. Manager, Care Management- Full Time, Days, 8 AM - 4 PM, Care Management, Atlantic Health Morristown Medical Center
Responsible for assisting both health care providers and facility supervisors. Ensures and assists care to the patients by matching the patient's needs with appropriate services. Maintains and updates patient records and manages business operations and patient care.
Principal Accountabilities:
1. Creates and updates personalized care plans for patients.
2. Collaborates with patients' families, friends, and social supports in developing treatment plans.
3. Identifies and recruits high-quality healthcare providers.
4. Interacts with healthcare providers to facilitate patients' treatments.
5. Ensures patients' treatment requirements are met, and suggests alternative treatment plans when patients' services requests do not meet medical necessity criteria.
6. Instructs and educates patients on procedures, healthcare provider instructions, and referrals.
7. Links patients to social services programs and entitlements such as transportation assistance and translation services.
8. Conducts regular follow-ups with patients to evaluate progress, promote continuity of care, and ensure improved health outcomes.
9. Performs other related duties as assigned.

Required:
1. Bachelor's degree in social sciences, social work, nursing,.
2. 2+ years of work experience in public health or related field.
3. Registered Nurse (RN) license.
Preferred:
1. Current case management experience.
