Basic Needs Case Manager
This position is located in Princess Anne, Maryland. Catholic Charities of the Diocese of Wilmington is seeking a Basic Needs Case Manager who will provide direct service and case management, ranging from one-time assistance to on-going support, for guests of Bethany House, a 24/7 homeless shelter, to provide referral services for access to community resources.
The schedule for this part-time position may vary and requires flexibility to work daytime, evenings, and weekends, based on operational needs, up to 28 hours per week.
Essential Functions:
- Provide case management services to support clients as they obtain housing stability, obtain earned or other income, and obtain health insurance and mainstream benefits so that clients can sustain housing on their own after the period of assistance. Assist individuals and families negotiate manageable and appropriate lease agreements with landlords.
- Maintain a caseload of open files including developing and monitoring service plans with each individual to meet basic needs focusing on stability of income, housing, self-care and other areas of functioning.
- Conduct intake interviews and determine eligibility pertinent to funding requirements giving full and detailed explanations to ineligible applications.
- Generate sound case decisions based on documented client information as to need and availability of funding and /or the need for access to other community resources.
- Travel locally to perform inspections for rental assistance, for home visits, other appointments as needed, as well as any outreach need that is identified and can be executed in a safe manner.
- Work with landlords to address potential barriers to participation and ensure correct understanding of the nature of the leasing agreement between the landlord and the resident.
- Refer clients to services such as employment services, housing counseling, budgeting. Assesses and target resources for those most likely to become homeless without assistance.
- Monitor community trends and potential issues within the program and provide input to Program Manager.
