Job Summary
Seattle Roots Community Health is seeking a Behavioral Health Care Coordinator – Community Based Programs to provide patient navigation, care coordination, community outreach, and program support across multiple community-based programs.
This position primarily supports three programs:
- STEP Clinic: A low-barrier substance use disorder treatment clinic co-located at Hepatitis Education Project.
- Health Care for the Homeless: A community-based program providing healthcare and outreach to individuals and families residing in Tiny House Villages, shelters, encampments, and other community settings.
- Reentry Program: A program supporting individuals transitioning back into the community following incarceration and assisting with connections to healthcare and other essential resources.
