The Role:
The Behavioral Health Case Manager provides phone support on the Agency’s Admissions Line and serves as a part of the Community Resource Center Department providing targeted case management to the clients for Adult Services, Youth and Family, or both teams. The Case Manager links clients wit h appropriate community service providers and organizations that empower individuals to live independently within their community.
What we’re looking for:
Bachelor's Degree required in human services or related field such as Social Work, Psychology, or equivalent experience
Social work or case management experience, in the areas of aging, disabilities, community health, nursing home or hospital discharge planning preferred
Experience in customer relations, call management, information and referral or related preferred
Excellent oral and written communication skills
Exceptional attention to detail
