Big Valley Tribal Case Manager

POSITION SUMMARY:

The Big Valley Tribal Court Case Manager is responsible for managing all cases in the Big Valley Adult Healing to Wellness Court and Youth Healing to Wellness Court programs. The Case Manager will work closely with the Tribal Court Judge, Court Manager, Healing to Wellness Court Team, Advisory Team, Tribal Police, Social Services, Education Department, and other court staff as hired. In addition, the Case Manager is responsible for working with individuals referred to the Wellness Courts, assesses their needs, coordinate services, and ensures compliance with program guidelines, including supervision, all while maintaining strict confidentiality. The Case Manager is under the supervision of the Big Valley Tribal Court Manager.

ESSENTIAL FUNCTIONS:

  1. Assess the needs of individuals referred to the Healing to Wellness Court using intake paperwork and individual interviews.
  2. Develop individualized plans to address those needs.
  3. Assist clients to accomplish goals as deemed by the case management plan, which may involve the supervision of activities.
  4. Ensure clients have the tools they need to be successful after the completion of the Wellness Court programs.
  5. Contacts, coordinates, and communicates with resources in the community including Tribal Social Services, Tribal Education, Tribal Housing, Tribal Administration, and Tribal Police.
  6. Complete home visits as necessary as based on the case management plan.
  7. Maintain accurate and timely records related to all clients.
  8. Coordinate access to various services, including treatment, counseling, and support programs.
  9. Ensure linkage and coordination among the court service providers.
  10. Monitor client compliance with program rules and court orders.
  11. Conduct, supervise, witness, and process drug tests.
  12. Advocate for the needs of clients within the Wellness Courts and with service providers.
  13. Maintain frequent contact with clients, service providers, and the Wellness Court Team.
  14. Meet with the Court Manager as necessary.
  15. Ensure effective communication and collaboration among all stakeholders.
  16. Maintain strict confidentiality of client information and complete the required Releases of Information as
    necessary.
  17. Transport clients as appropriate and/or assist clients with the use of public transportation.
  18. Attend all case management meetings and court sessions; report on client progress and perform follow-up to
    assure decision implementation.
  19. Provide relapse prevention support and activities to clients.
  20. Engage with clients using non -adversarial approach.
  21. Interact respectfully and collaboratively with all team members and external partners.
  22. Participate in all required trainings.
  23. Conduct data collection and entry.
  24. Provide immediate support for clients experiencing crises related to substance use, mental health, or legal issues.
  25. Conduct ongoing evaluations to adjust case plans based on progress or setbacks.
  26. Assist clients with long-term success strategies after program completion, including employment, housing, and continued treatment.
  27. Ensure continuity of care for justice-involved clients.
  28. All other duties as assigned.
