Guardian Angel Senior Services is seeking a compassionate, organized Registered Nurse (RN) to join our home care team. Bring your nursing knowledge, communication skills, and commitment to quality care to a role focused on client assessments, caregiver education, and care coordination.

What You'll Do

• Conduct client assessments and identify care needs.

• Develop, review, and update individualized care plans.

• Monitor changes in client condition and communicate concerns promptly.

• Provide guidance and support to caregivers, clients, and families.

• Ensure caregivers follow care plans and agency requirements; provide re-education as needed.

• Assist with caregiver training, competency reviews, and ongoing education.

• Maintain accurate, timely nursing documentation.

• Collaborate with the office team to support safe, consistent care.

What We're Looking For

• Current Massachusetts Registered Nurse license.

• Strong assessment, communication, and organizational skills.

• Ability to work independently and collaborate with a team.

• A patient, practical approach to educating caregivers and families.

• Reliable transportation for visits in Billerica and surrounding towns.

• Home care, visiting nurse, geriatric, or long-term care experience preferred.
