What You’ll Be Doing:

As a Care Coordinator, you’ll be the backbone of our client care process, ensuring that our consumers and their Personal Care Assistants (PCAs) receive the support they need. Your responsibilities will include:

• Manage a Caseload: Oversee an assigned caseload of consumers and PCAs, making sure that each client’s care plan is up-to-date and accurately reflects their needs.
• Conduct Assessments: Perform in-home and telephonic assessments to identify and address individual care needs. Update care plans every 90 days or as needed.
• Ensure Quality Care: Monitor and verify PCA visit times and care provided, ensuring compliance with care plans and regulatory guidelines.
• Document and Report: Keep meticulous records of all assessments, wellness calls, and other consumer interactions in the HHA Exchange system. Ensure all documentation is completed within 48 hours.
• Provide Training: Educate PCAs on the EVV system and other processes as needed to ensure seamless service delivery.
• Resolve Issues: Address any concerns from consumers or PCAs, escalating issues to your manager when necessary.
• Coordinate Services: Adjust schedules, confirm service authorizations, and communicate any changes to both consumers and PCAs to ensure optimal care delivery.

What You Bring to the Table:

• Expertise: A basic understanding of medical conditions and terminology, along with experience in healthcare or home care environments.
• Communication: Strong verbal and written communication skills that allow you to interact effectively with clients, caregivers, and healthcare professionals.
• Organizational Skills: The ability to manage your time, prioritize tasks, and meet deadlines in a fast-paced environment.
• Problem-Solving: A knack for identifying issues and finding effective solutions quickly.
• Customer Service: A passion for providing top-notch service and advocacy for your clients.
• Tech Savvy: Proficiency in Microsoft Office and the ability to quickly learn and navigate the HHA Exchange system.

Required Qualifications:

• Education: A minimum of an Associate’s degree in a related field.
• Experience: At least 5 years of experience in healthcare or home care preferred.
• Licensing: A valid driver’s license with daily access to a reliable, registered, and insured vehicle.
• Language Skills: Spanish language skills are a plus, but not required.
• Physical Requirements: Ability to occasionally lift objects between 10 and 25 lbs. and perform other physical tasks as needed.
