Position Summary:
As the Bilingual RN, you would be responsible for supporting SNP (Special Needs Plan) efforts for immediate member outreach to complete Health Risk Assessments and Care Plans.
Hours for this Position: Monday-Friday (40 hours/week)

More Insight of Daily Responsibilities:
- Complete HRA questionnaire to provide individuals with an evaluation of their health risks and quality of life
- Oversee patient care plans/unique medical needs
- Provide care to individuals that tailor to their benefits and provider choices
What We Look For:
- Clear and Active RN License in state of FL
- Case Management Experience
- HRA Experience
- SNP Experience
- Bilingual in English/Spanish or Creole
