Campus RN

The Campus Home Care Nurse is responsible for assisting in the overall planning, coordination, provision, supervision and evaluation of all clinical services for Osborn Home Care, within The Osborn’s Campus. As a representative of Osborn Home Care, and of the organization, it is required that the Community/Campus Home Care Nurse perform the essential functions of the job in a professional manner, with a positive attitude and is aware and supportive of the mission, vision, resident Bill of Rights, policies and procedures of the organization.

Responsibilities include:

• Assisting with the coordination of all health related services for the client/family.
• Performing initial assessments of newly referred clients to assess care needs.
• Obtaining appropriate and timely initial and updated physician orders.
• Developing and implementing Plans of Care and treatment plans in conjunction with physician orders.
• Providing ongoing teaching to clients and family as needed to support health and wellness outcomes.
• Updating medical record as appropriate.
• Supervising HHAs.

Qualifications:

• Must have NYS RN License and be CPR Certified
• BSN preferred
• 3 - 5 years experience in home care.
• Must have ability to work with others.
• Computer literacy and knowledge of EMR
