Job Scope

A. Programme Development and Implementation

i. Assist in running of targeted outreach and cognitive stimulation activities.

ii. Support in initial assessment of walk-ins and referral cares, follow-up intervention plans for clients as per the prescribed care plan and under the guidance of Allied Health Professional

iii. Support in the monitoring the client's condidtions and progress and escalate to Allied Health Professional where necessary

iv. Plan and conduct outreach to the community and stakeholders to promote mental wellness and mental health awareness, including community mapping

v. Assist in executive of structured preventive activities and/or programmes for the community, those at risk of mental health conditions and clients.

B. Administration

i. Responsible for maintaining client records that are accurate and timely for submission to AIC for data analysis, in accordance with the CREST programme requirement.

ii. Keep and maintain client and programme data and documents, in accordance with the CREST programme requirement, as well as the prevailing personal data protection and security guidelines.

Candidate Requirements
Diploma in Social Work, Counselling, Psychology, or other fields related to healthcare and community care

Preferably 2 - 3 years of healthcare/community care experience

· Experience in the health and social services sector, particularly in caring for persons with mental health

· Strong interest in advocating mental wellness

· Possess good interpersonal skills, proactive in outreach

· Highly motivated and service mindset to serve the community

· Able to work independently and a team player

· Good time management, efficient and able to multi-task
