Care Associate (Sheltered Home)

Job Responsibilities

  • Ensure healthcare and daily living needs of residents in the Home are well taken care of to promote independent living of residents and to enhance their overall quality of life.
  • Provide person-centric care to residents through basic nursing care procedures under the direction of Staff Nurse (SN) and/or Enrolled Nurse (EN), and supporting residents’ general well-being and living conditions.
  • Provide basic physiological monitoring of residents on a monthly basis and report any health issues to SN/EN for follow-up.
  • Conduct daily vital signs monitoring for all residents and ensure timely and accurate documentation.
  • Assist residents in their activities of daily living and instrumental activities of daily living to maximise their independent living skills.
  • Perform medication packing, under the supervision of SN/EN, and medication reminders for residents with higher care needs.
  • Respond timely to alert alarms, medical emergencies or crisis situations to provide first aid and activate the Police and/or Emergency Medical Services.
  • Carry out night shift duties on rotational basis to ensure emergency situations of residents are attended to.
  • Conduct night roll call during night shift duty to check for the safe return of all residents back to the Home.
  • Conduct routine checks to maintain the general upkeep, cleanliness, and prevention of hoarding in resident units.
  • Escort residents to hospitals and polyclinics for medical appointments whenever necessary.
  • Assist with general upkeep duties to support in the care of residents, such as preparation of meals and laundry.
  • Protect residents’ confidentiality by adhering strictly to organisational Personal Data Protection Policy and national Personal Protection Data Act (PDPA) and regulations.
  • Adhere to organisational and service Standard Operating Procedures and service requirements at all times.
  • Work with the Social Work and Programmes teams to plan and conduct social and/or educational activities to promote residents’ independence, health wellness and acquisition of social skills.

Requirements

  • Minimum Secondary Education
  • Able to read, write and speak basic English
  • Able to stay-in and perform night shifts on rotational basis
  • Passionate to help the elderly
  • Able to work independently
  • Attention to details
  • Background knowledge on nursing care and/or basic first aid will be an advantage
