Coordinates a youth-focused healthcare program, ensuring services meet agency and state health regulations while prioritizing family-driven care.
The Program Coordinator is responsible for the day-to-day management of the Care Management program including assuring high-quality, family-driven and youth-guided service in accordance with the policies and regulations of the agency, the Health Homes Serving Children (HHSC), and the NYS Department of Health (DOH).
