Care Management RN
Day Shift, Tuesday - Friday; 32 hours per week
- Graduate of an approved accredited registered nurse (RN) program
- Current RN license in the State of Michigan, in good standing
- Current BLS certification or recertification within 3 months of hire
- Will obtain and maintain Care Manager Certification within 6 months of working in the program
- Must exhibit a high level of human relation skills and communication skills
- Must have a general knowledge of nursing protocols, procedures, safety procedures and requirements
- Education and/or experience with chronic disease and self-management education preferred
