Care Manager
Position Summary:
The Care Manager is a leadership position responsible for overseeing daily operations and the performance of the caregiving team. This role ensures shift efficiency, safety, and continuity of care by serving as the primary link between hospice providers and the caregiving team. By balancing active team mentorship and administrative oversight, the Care Manager ensures that care plans are effectively implemented and that the Home’s operational standards are upheld.

Schedule & Availability
- We operate on a Monday–Friday schedule with consistent shift windows (Day Ops: 6 AM–6 PM / Night Ops: 6 PM–6 AM).
- This role includes a shared on-call rotation and occasional weekend/holiday coverage.
- We prioritize a balanced 40-hour week. When on-call or weekend duties arise, we utilize flexible scheduling to offset those hours, ensuring you stay refreshed and energized.
Qualifications & Skills:
- Two or more years of experience within the caregiving, hospice, or long-term care industry
- One or more years in a supervisory or lead role, with a proven ability to mentor staff and foster a team environment
- Excellent verbal and written skills; ability to present ideas clearly and constructively
- Strong ability to multitask, work independently, and meet deadlines
- Proficiency in Microsoft Office Suite

- Empathy for residents and families navigating end-of-life transitions and the emotional resilience to lead in a hospice setting
Supervisory & Operational Duties
- Lead the full onboarding and training process to ensure staff success. Foster a culture of accountability through ongoing mentorship, scheduled performance reviews, and the professional management of corrective actions and retraining as needed.
- Direct shift operations to ensure resident safety and consistent continuity of care.
- Serve as the primary point of contact for hospice teams; coordinate care plan changes, discuss care concerns, and ensure all external partners adhere to the Home’s policies and operational standards.
- Monitor inventory and provide timely reorder requests to the Director of Programming.
- Manage the monthly Caregiver schedule, including publishing shifts, processing swap requests, and reviewing and submitting payroll.
- Assist in preparing and facilitating mandatory quarterly all-staff meetings, bi-weekly leadership meetings, and program-specific committees.
- Provide direct care on the floor as needed and assist with other administrative projects as assigned by leadership.
Additional Information:
Employment is contingent upon completing a drug screening, TB test, and background check. While CNA certification and fluency in a second language are not required, they may enhance your effectiveness in this role.
This is a non-exempt position reporting to the Director of Programming.
