Care Navigation Coordinator

Coordinates care plans and services for children with intellectual and developmental disabilities, managing assessments, referrals, and clinical records in a healthcare setting.
The Care Navigation Coordinator works with children and families enrolled in program serving intellectually and developmentally disabled children ages 0-17. The Supports Coordinator is identified as the chairperson of the multi-disciplinary team and the developer of the Person-Centered Plan. They are responsible for assessment, referrals, linking and coordination of all services, as well as for monitoring and supervising prescribed services. They are also IDD Services Facilitator is also responsible for the clinical record.
