Caregiver Simi Valley 12 hour shifts Sunday, Friday and one night shift TBD hours are 9AM-9PM and 9PM- 9AM

Introduction:

Family Ties Home Care has a client in Simi Valley who has early Alzheimer's and has 24 hour help. She is a fall risk. She is very independent and strong willed but has come to appreciate the help.

Responsibilities:

• Following a pre-set care plan
• Standard ADLs (personal care, bathing, dressing, grooming, hygiene assistance)
• Assist with medication reminders
• Monitor and report on client's well-being
• Light housekeeping, laundry, & other household daily chores as needed
• Grocery shopping & errands
• Cooking/meal preparation
• Scheduling appointments, including doctor's
• Communication with Family Ties HC management & family members/loved-ones regarding the client's daily status
• Daily notes
• Other duties assigned as needed

Qualifications:

• Current HCA or higher certification
• Caregiving experience of 1 year or similar experience
• Compassionate and caring demeanor
• Excellent communication & interpersonal skills
• Ability to follow the pre-set care plan and medication schedule
• Ability to lift 35 pounds
• Ability to assist with client's mobility as needed
• Valid driver's license and reliable transportation
• Ability to work the schedule as described
