Summary
Provide community-based case management and crisis support to clients in behavioral health, conducting most visits in person and maintaining detailed documentation.

Status: Full-time
GENERAL RESPONSIBILITIES:
- Provide Case Management (CPST, Crisis) to persons assigned to caseload.
- Provide goal-directed support and solution-focused interventions.
- Work hours are flexible to meet the needs of the person served.
- Will make all person contacts as indicated in tx plan.
- The majority of contacts happen out of the office and in community settings.
- Contact with new referral is made at opening meeting or within 24 hours of opening meeting.
ESSENTIAL JOB FUNCTIONS:
- Maintains contacts assuring persons receive contacts as designated in Treatment Plan and if going to be out of the office, makes arrangements with other CM staff to ensure contact continues.
- Weekly contact sheets turned in at the end of the workweek.
- Progress notes complete within 48 hours of service.
- Attend tx plan meetings every 90 days and following a crisis.
- Complete DLA20 every 90 days and help coordinate CBCL’s are completed timely.
- EMR scheduler is utilized and kept updated.
