Case Manager

Since 1957, we’ve assisted a wide array of populations throughout Southwestern Pennsylvania, focusing on those generally underserved by traditional services including senior citizens, veterans, individuals with physical and/or intellectual disabilities, individuals on the autism spectrum, and youths who have aged out of foster care.

JOB SUMMARY

Position will work with both Rapid Re-Housing and Permanent Supportive Housing Program (as needed). MyPlace offers up to 24 months of Rapid Re-Housing for homeless youth and youth aging out of foster care. Permanent Supportive Housing works with homeless youth who have a dual diagnosis. Case Managers will engage in intensive case management services with their clients to help guide them to achieve self-sufficiency and permanent housing.

ESSENTIAL FUNCTIONS
  • Provide case management duties including school, employment, referrals
  • Maintaining a physically and emotionally safe environment
  • Meet with clients in their units, in the office, and out in the community several times a month
  • Developing and maintaining positive relationship with clients
  • Help to secure housing for program participants by visiting apartments throughout the Pittsburgh area and ensuring their cleanliness and safety
  • Completion of the HMIS assessment on new clients within program time frames and update assessments as client’s information changes
  • Develop, monitor, and revise client goal plans
  • Develop a discharge plan to include a two-month aftercare plan services for program graduates
  • Maintain and update all clients’ files per regulations under DHS regulations
  • Document activities of casework and interaction with clients through written case notes
  • Help in moving participants into their apartments
  • Teach and model independent living skills
  • Creating an inspiring team environment through open communication
  • Agencies
    1. Establish/maintain relationships with agencies in Allegheny County.
    2. Establish/maintain relationships with other service providers as needed to discharge job responsibilities.
  • Reports
    1. Help prepare all reports as required by funding agencies.
  • Other duties as assigned.
JOB REQUIREMENTS AND EXPERIENCE
  • Bachelor’s Degree in Social Services preferred
  • Prior experience working with youth is required

  • Physical strength to manipulate light to medium weight materials weighing 10 to 15 pounds on a regular basis
