Description

Case Manager

RLC-SUMNER,WA

We are a nationwide network of addiction treatment centers providing evidence-based care for substance use disorders and co-occurring mental health conditions. Through individualized treatment and a full continuum of care, we help clients build the skills, confidence, and foundation needed for lasting recovery and a healthier, more fulfilling life.

About the Role

As a Case Manager, you are responsible for helping clients manage the practical side of their lives so they can focus on treatment. From admission through discharge, you will be a consistent point of contact for your clients, identifying their needs, communicating with the treatment team, and coordinating the outside pieces that get in the way of recovery, such as court dates, family communication, finances, housing, transportation, and employment. You will also conduct initial needs assessments, keep timely and accurate documentation of every case management interaction, and support discharge and aftercare planning, including referrals to sober living, outpatient, and step-down programs.

This is a hands-on role that touches every level of care. You will work with detox, residential, and IOP clients, coordinate therapy, medical, and psychiatric appointments, provide warm handoffs as clients move between levels of care, and facilitate psycho-educational groups as needed. You will take part in weekly clinical team meetings under the oversight of the Clinical Director, support intake and admissions, and serve as a resource to families looking for guidance and referrals.

  • Coordinate and track all residential client appointments and ensure appropriate staff are informed.

  • Conduct initial needs assessments for detox clients and coordinate immediate services (e.g., medical follow-up, family communication).
