Case Manager

We are seeking a dedicated Case Manager to join our team. As a Case Manager at our Counseling Clinic, you will play a crucial role in providing comprehensive support to patients as they navigate their healthcare journey. Your primary responsibility will be to assess, plan, implement, coordinate, monitor, and evaluate the options and services required to meet the health and psychosocial needs of our clients. You will work closely with a multidisciplinary team to ensure seamless coordination of care, enhance patient outcomes, and optimize their overall well-being.

Essential Functions

1. Assessment and Planning:

· Conduct thorough assessments of patients' health, psychosocial, and environmental needs.

· Collaborate with patients, families, and healthcare providers to develop individualized care plans.

· Identify resources and services required to address patients' needs effectively.

2. Coordination and Monitoring:

· Serve as the primary point of contact for patients, families, and external service providers.

· Coordinate appointments, referrals, and follow-up care to ensure continuity of care.

· Monitor patients' progress and adjust care plans as necessary to achieve desired outcomes.

3. Advocacy and Support:

· Advocate for patients' rights and preferences within the healthcare system.

· Provide emotional support, counseling, and education to patients and families.

· Assist patients in accessing community resources and social support networks.

4. Documentation and Reporting:

· Maintain accurate and up-to-date documentation of all patient interactions and interventions.

· Prepare reports and summaries as required for internal and external stakeholders.

· Ensure compliance with regulatory requirements and organizational policies.

5. Collaboration and Teamwork:

· Work collaboratively with physicians, nurses, social workers, and other healthcare professionals to achieve shared goals.

· Participate in interdisciplinary team meetings to review patient cases and develop care plans.

· Contribute to the continuous improvement of clinical processes and patient care outcomes.

6. Education and Outreach:

· Provide education to patients and families regarding their diagnoses, treatment options, and self-care strategies.

· Conduct outreach activities to promote awareness of available services and resources in the community.

· Facilitate support groups and educational workshops as needed.

Work Environment

This position works in an outpatient counseling clinic. There will be frequent interaction with persons who may be homeless, mentally ill, and emotionally upset.

Physical Demands

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. The employee must have the ability to lift, pull, push, and manipulate equipment and clients, which requires strength and gross motor and fine motor coordination. The employee must be able to administer prescribed treatments and perform CPR and First Aid. The position requires sitting and/or standing for long periods of time.

Position Type/Expected Hours of Work

This is a full-time position. Days and hours of work are typically Monday through Friday, 9 a.m. to 4 p.m.

Required Education and Experience

1. Bachelor’s degree.

2. Previous experience in case management, preferably in an outpatient healthcare setting.

3. Strong interpersonal, communication, and problem-solving skills.

4. Ability to work independently and as part of a multidisciplinary team.

5. Knowledge of healthcare systems, community resources, and regulatory requirements.

6. Empathy, compassion, and a commitment to advocating for vulnerable populations.

7. Proficiency in computer applications and electronic health records.


Preferred Education & Experience

1. Master's degree in social work, nursing, psychology, counseling, or a related field.

Additional Eligibility Qualifications

· CPR and First Aid

· Valid Arizona Driver’s License.

· Must pass a TB test and TB screening annually.

· 21 years of age or older.

· Fingerprint clearance card.
