Position Statement:

The Case Manager plays a key role in the coordination, management, documentation and support of the treatment and programming of youth within our care. Of equal importance is the Case Manager’s role of liaison, developing and maintaining rapport and communication with the county, family and the youth. The Case Manager is a full-time benefit eligible position that is scheduled in a manner that best accommodates the individual and case-by-case needs of treatment programming. Work weeks in excess of 40 hours and weekend hours can be occasionally anticipated. All overtime hours requires pre-authorization. The Case Manager is expected to have safe and reliable transportation and use their personal vehicle for case management business when Agency vehicles are not available. Work-related miles driven with a personal vehicle will be compensated at the Agency’s published reimbursement rate.

Responsibilities:

  • The Case Manager collaborates and communicates with county workers, service providers, program staff and family members; ensuring that the exchange of information, logistical support, and resolution of concerns between the parties are brought to the forefront and managed to an appropriate outcome, while also advocating for a youth’s best interests.
  • Communicate and consult regularly with the Clinician with a focus on transferring the clinical implications and goals of treatment to the family, county and program.
  • Collaborate with departments within the agency to ensure congruency of services and treatment.
  • Attend all required meetings deemed necessary by the Case Manager Supervisor.
  • Maintain all required record keeping, reports and documentation in accordance with Agency and State policies and procedures.
  • Provide direct support to families where needed (visitation needs, home assessments etc.)
  • Collaborate with the program management team when treatment, program planning, paperwork and intra-agency communication dictate.
  • Professionally represent the organization from the standpoint of its values, mission and its treatment initiatives and philosophy.
  • Consistently demonstrate and promote principles of Agency wide initiatives, notably those of the Therapeutic Crisis Intervention (TCI), Boys Town Psychoeducational Model (PEM), and other skills-based models.
  • Understand and adhere to all Agency policies, protocols and procedures.
