CASE MANAGER POSITION DESCRIPTION

Position Information

Position Title: Case Manager

Department: Housing and Supportive Services

Reports To: Program Director FLSA Status: Exempt Employment Status: Full-Time

Position Summary

The Case Manager provides trauma-informed, youth-centered case management services to young people experiencing homelessness or housing instability. The position supports youth in achieving housing stability, educational attainment, employment success, life skills development, and long-term self-sufficiency.

Through individualized service planning, advocacy, crisis intervention, and resource coordination, the Case Manager assists youth in identifying and achieving personal goals while fostering independence and positive community engagement. This position serves as a key liaison between Covenant House Pennsylvania, community partners, landlords, service providers, and program participants.

Essential Functions and Responsibilities

Case Management and Service Planning

  • Conduct comprehensive assessments to identify client strengths, needs, barriers, and goals.
  • Develop, implement, monitor, and regularly update individualized service plans in partnership with youth.
  • Provide ongoing case management, coaching, advocacy, and support to assist youth in achieving program goals.
  • Assist youth in securing and maintaining stable housing, employment, educational opportunities, healthcare, and community resources.
  • Monitor client progress toward goals and adjust service plans as needed.
  • Facilitate life-skills development in areas such as budgeting, employment readiness, communication, conflict resolution, health, and independent living.

Youth Engagement and Support

  • Build positive, supportive, and professional relationships with youth through a trauma-informed and strengths-based approach.
  • Provide crisis intervention, de-escalation, and problem-solving support when needed.
  • Conduct regular check-ins, home visits, community visits, and supportive counseling within the scope of the position.
  • Escort youth to appointments, interviews, meetings, court appearances, or community activities when appropriate.
  • Encourage participation in program activities and community engagement opportunities.

Housing Stability and Advocacy

  • Assist youth in locating, obtaining, and retaining safe and stable housing.
  • Advocate with landlords, housing providers, employers, educational institutions, and service agencies on behalf of youth.
  • Support housing placement, lease compliance, and conflict resolution efforts.
  • Monitor housing stability and intervene proactively to prevent housing loss.

Documentation and Compliance

  • Maintain accurate, timely, and comprehensive case records in the agency's electronic data management system (ETO or successor platform).
  • Document all client contacts, assessments, service plans, referrals, progress notes, and case activities in accordance with agency, contractual, and regulatory requirements.
  • Complete all required grant, program, housing, and outcome reporting accurately and within established deadlines.
  • Ensure confidentiality and compliance with HIPAA, agency policies, and applicable regulations.

Program Administration

  • Participate in youth screening, intake, assessment, and eligibility determination processes.
  • Participate in case conferences, multidisciplinary team meetings, staff meetings, and training sessions.
  • Assist with the responsible allocation, tracking, and documentation of program resources and petty cash in accordance with agency policies.
  • Represent Covenant House Pennsylvania at community meetings, coalitions, and partner agency events as assigned.
  • Contribute to program development initiatives and continuous quality improvement efforts.

Professional Responsibilities

  • Maintain professionalism and ethical conduct in all interactions with youth, staff, volunteers, community partners, and stakeholders.
  • Maintain availability and flexibility to respond to client needs and program demands.
  • Participate in required trainings and professional development activities.
  • Perform other duties as assigned to support agency operations and client success.
