Full and Part Time position available

Position Summary:

The CIC Case Manager is an integral member of the interdisciplinary treatment team, providing case management services to support patients throughout their stay at the Crisis Intervention Center and facilitate a safe and successful transition back to the community. The Case Manager works collaboratively with patients and the treatment team to identify individualized goals, needs, barriers, and resources necessary to support stabilization and continued recovery. A primary focus of the position is discharge planning and continuity of care, including coordination of follow-up behavioral health and medical appointments, access to prescribed medications, housing and basic-needs stability, transportation, benefits and community resources, and other services necessary to support the patient following discharge. The Case Manager communicates and collaborates with families, community providers, and other involved supports, as appropriate, to reduce gaps in care and promote successful transition from the CIC.

Primary Job Duties and Responsibilities:

  • Participate as an active member of the interdisciplinary treatment team, collaborating with the Clinical Director, nursing staff, medical providers, and other CIC staff to identify patient needs, goals, barriers, and priorities for stabilization and discharge.
  • Meet with patients following admission to assess case management and discharge needs, including behavioral health and medical follow-up, medication access, housing, transportation, financial or benefits concerns, social supports, and other factors that may affect a successful transition from the CIC.
  • Participate in the development and implementation of the individualized treatment plan, working with patients and the treatment team to establish achievable goals and identify services and supports necessary during and following the CIC stay.
  • Initiate discharge planning early in the patient’s stay and continually update the discharge plan as patient needs, circumstances, and treatment recommendations change.
  • Coordinate follow-up behavioral health, substance-use, medical, and other necessary services, including scheduling or confirming appointments whenever possible and communicating relevant transition information to receiving providers in accordance with applicable confidentiality requirements.
  • Promote continuity of prescribed medications following discharge by working with nursing and medical providers to identify and address barriers related to prescriptions, medication access, pharmacy coordination, insurance or payment, and other medication-related needs.
  • Assess and address housing and basic-needs stability, including identifying safe discharge destinations and connecting patients with shelters, housing resources, food assistance, transportation, financial resources, benefits, and other community supports when needed.
  • Assist patients in accessing public benefits, insurance coverage, financial assistance, identification documents, and other resources when these needs present a barrier to continued treatment, medication access, housing stability, or successful discharge.
  • Collaborate with family members, legal guardians, outpatient providers, community mental health centers, hospitals, substance-use treatment providers, housing programs, and other community partners, as appropriate and authorized, to support continuity of care and successful transition.
  • Advocate for patients and assist in resolving barriers to services, helping patients understand available options and supporting their ability to make informed decisions regarding treatment, recovery, and post-discharge needs.
  • Provide patients with clear discharge information and resource connections, ensuring they understand follow-up appointments, referrals, available community supports, and the next steps identified in their discharge plan.
  • Communicate patient progress, identified barriers, resource needs, and discharge readiness to the treatment team and participate in treatment-team meetings, case discussions, and other interdisciplinary planning activities.
  • Complete accurate and timely case management documentation, including assessments of identified needs, patient goals, interventions, referrals, contacts with outside providers or supports, appointments, resources arranged, barriers encountered, and discharge-planning activities.
  • Maintain patient confidentiality and protect patient rights in accordance with HIPAA, 42 CFR Part 2 when applicable, CIC policies, and other applicable state and federal requirements.
  • Participate in required CIC orientation, training, staff meetings, and emergency-preparedness activities and maintain competency in trauma-informed care, crisis intervention, patient rights, confidentiality, and other areas required for the position.
  • Develop and maintain professional, collaborative relationships with patients, families, CIC staff, community providers, and other partners; follow Center policies and procedures, accept supervisory direction, and perform other related duties as assigned.
