Job Summary:
Provide financial, housing, and supportive services to Transitional Aged Youth (18-24) who are at-risk of or currently experiencing homelessness. Administer monthly financial assistance accompanied by prevention, diversion, and housing solution strategies to support youth in locating and securing permanent housing while also advancing their progress in education and employment goals.
Essential Duties/Responsibilities:
- Provide individual, face to face case management to 20-25 Transition Age Youth who are enrolled in the Direct Cash Program
- Case Management may include:
- Assist participants to reach their academic goals
- Provide training in daily living skills, budgeting, locating and maintaining housing and career plannin
- Collaborate with participants to incorporate their goals into the Case Management plan and assist youth in achieving all goals
- Provide opportunities, connections and resources for affordable, safe housing and transitional housing programs at BWC or within the community
- Provide opportunities for support and participants who are parenting
- Distribute financial incentives for participants
- Provide resources and referrals to relevant, culturally appropriate services in the community where the youth resides or plans to reside
- Coordinate with community providers to refer participants to programs and services needed
- Provide continuous outreach to engage youth not actively participating in services.
- Attend required agency and stakeholder meetings.
- Maintain client files, statistical data and reports per protocol and deadlines.
- Coordinate college/career exploration tours.
- Participate in agency Program Quality Improvement activities and initiatives
- Participate in agency Safety initiatives and proactively report safety issues to management immediately
- Regular attendance is required.
- Other duties may be assigned.
