Case Manager- Guadalupe Home

Summary:

Case Manager is responsible for implementing day-to-day and long-term case management services to residents of Guadalupe Home, both in the home and out in the community when applicable. Assess needs of each resident and her child, assisting in the development of comprehensive service plans for the assigned residents. This role also provides case management services to clients in an integrated, multi-cultural setting. The Case Manager will work with community resources and other community-based providers/representatives to coordinate, monitor, evaluate, and advocate for services to meet each resident’s specific needs in tracking performance measures and goal path created for each resident.
