What You Will Do
- Collaborate with the healthcare team to identify and assess patient needs.
- Develop and implement care plans that promote patient safety and quality of care.
- Coordinate services and resources to support patients' health outcomes.
- Provide effective discharge planning to ensure a safe transition to the next level of care.
- Advocate for patients and families, addressing their needs and concerns.
- Evaluate care plans and outcomes to optimize health along the care continuum.

What You Will Need
Required Qualifications
- Bachelor's degree in nursing or a Master's degree in Social Work.
- Minimum 3 years of clinical experience.
- Excellent verbal communication and written documentation skills.
- Registered Nurse (RN) or Licensed Social Worker (LMSW) with a valid, unrestricted State of Michigan license.
Preferred Qualifications
- Certification in Case Management (CCM) by the Commission for Case Management Certification (CCMC).
- Experience in discharge planning, home health care, rehabilitative medicine, or managed care.
