What You’ll Do
- Build trusting, person‑centered relationships with individuals and families experiencing homelessness
- Maintain an assigned caseload and meet with clients regularly through in‑person and creative outreach
- Complete strength‑based assessments and partner with clients to create housing‑focused goal plans
- Support clients while they are in shelter or temporary settings to prepare for housing opportunities
- Assist clients in locating, obtaining, and maintaining scattered‑site housing
- Coordinate services related to medical care, mental health, substance use, income, and benefits
- Support housing stability through landlord communication, eviction prevention, and mediation when needed
- Help clients understand tenant rights and responsibilities
- Document services and housing activities accurately and on time in required systems
- Collaborate closely with teammates, supervisors, and community partners
- Practice Avivo’s person‑centered, trauma‑informed, harm‑reduction, and anti‑oppression approaches
