Case Manager I - TCM

As a Case Manager, you’ll support adults living with mental illness by building strong relationships, coordinating care, and helping clients work toward greater stability and independence. You’ll develop individualized treatment plans and connect clients with the resources and services they need to succeed in the community.

What You’ll Do

• Build trusting relationships and develop individualized treatment plans.
• Provide supportive counseling, psychosocial rehabilitation, and symptom monitoring.
• Provide crisis support and intervention when needed.
• Monitor psychiatric medications as directed by the physician.
• Coordinate care with clients, families, healthcare providers, and community resources.
• Connect clients with housing, benefits, financial resources, and other supportive services.
• Help clients develop skills to maintain housing and increase independence.
• Coordinate services among professionals and providers involved in each client’s care.
• Support clients through transitions and develop appropriate discharge plans.
• Maintain accurate and timely documentation and treatment records.
• Participate in team meetings, training, quality improvement, and community activities.
• Provide coverage and support for other team members as needed.
