Case Manager

As a member of the team it is your opportunity to play a vital role in providing wellness coaching, care coordination and support navigating services to children, youth and families. You will help connect individuals to essential resources and care, making a meaningful impact everyday. You will deliver ongoing community based assessment, treatment and supports for children and youth experiencing or at-risk for severe emotional disturbance assisting as they and their families traverse healthcare, community systems and education systems.

Summary of Duties

  • Identify youth and family needs, strengths and goals while simultaneously addressing barriers to accessing services
  • Make referrals and follow up on services
  • Support communication between providers and members of the youth’s team
  • Support youth with goal-setting, organization, problem-solving and self-advocacy
  • Provide support in schools, homes, and community settings
  • Document services, referrals, coordination, and progress
  • Collaborate with clinicians and other CYFS staff

The starting pay range for this position is $65,124 to $68,452 annual salary.
