Case Manager – Lee County

In this role, you'll work directly with older adults throughout the community, assessing their needs, developing individualized care plans, and connecting them with essential services that help them remain safe, healthy, and independent in their own homes. This position meets with seniors in their homes to complete assessments in person. Other related work can be completed remotely from home. Candidates must be live in Lee County Florida and be comfortable driving their personal vehicle (mileage reimbursement included).

Essential Functions:

  • Assess the needs of older adults who have been referred to Senior Friendship Centers and screened as possible recipients of Title III, Community Care for the Elderly, Alzheimer's Disease Initiative, Home Care for the Elderly, and other programs.
  • Perform an in-home assessment with clients to evaluate the impact of services.
  • Assess, plan, implement, and monitor service delivery plans with clients utilizing community resources. Perform required follow-up evaluations of services.
  • Complete the Department of Elder Affairs (DOEA) required annual assessment of each client.
  • Meet productivity goals as outlined by the Supportive Aging Services Director.
  • Complete client reports, logs, and case file documentation per program requirements.
  • Remain current on DOEA grant program guidelines, including attendance at training programs.
  • Participate in fundraising and outreach activities to promote the organization’s mission.
  • Report any incident of abuse or neglect as mandated by state guidelines.
  • Must comply with HIPAA regulations and OSHA laws and safety guidelines.
  • Attend staff meetings, training, and seminars as required.
  • Must comply with all Senior Friendship Centers personnel policies.
  • Must protect the safety and health of staff, volunteers, participants, and others by observing and following the organization’s safety and incident policies.

  • None.
