Why This Role Matters

Every day, individuals experiencing homelessness face barriers to housing, healthcare, mental health services, and basic needs. As a Case Manager, PATH Navigation, you will meet people where they are, build meaningful relationships, and help connect them to the resources and support needed to move toward housing stability and improved wellness.

Working throughout the community, you'll provide outreach, housing navigation, and case management services for individuals living with serious mental illness and/or co-occurring substance use disorders. This role is ideal for someone who is passionate about relationship-based work, creative problem-solving, and helping people achieve meaningful change in their lives.

What Makes This Position Unique

  • Full-time pay and benefits with a 32-hour work week
  • $5,000 Hiring Bonus Available! Eligible employees receive $1,000 on their first paycheck and $4,000 after one year of employment, subject to eligibility requirements and continued employment

What You'll Do

  • Provide housing-focused outreach and case management services to individuals experiencing homelessness
  • Build trusting relationships through consistent, person-centered engagement and support
  • Assist participants in obtaining identification, benefits, healthcare, behavioral health services, income resources, and housing opportunities
  • Conduct community-based outreach in shelters, encampments, homes, and other nontraditional settings
  • Support participants in locating housing and maintaining housing stability
  • Partner with community providers, Coordinated Entry, and housing systems to remove barriers and increase access to services
  • Provide crisis intervention, safety planning, advocacy, and support during challenging situations
  • Develop individualized service plans and help participants achieve housing and wellness goals
  • Maintain timely and accurate documentation in HMIS, Credible, and other required systems
  • Promote Housing First, Wellness & Safety, Trauma-Informed Care, and person-centered practices throughout all services
