We are seeking a Community Case Manager / Prevention Specialist who enjoys building relationships with young people, working collaboratively with families and community partners, and helping youth develop practical skills for success.

This is a position where no two days look exactly the same. You may meet individually with a young person working toward personal goals, help a teen develop independent living skills, facilitate a prevention program in a school, connect a family with community resources, or collaborate with a therapist, caregiver, or school team.

Job Snapshot

Schedule: Monday–Friday with some afternoon/evening hours based on program and client needs
Work Setting: Schools, homes, community locations, and Pathway programs
Travel: Local travel required; reliable transportation and appropriate auto insurance required
Training: Paid training, ongoing supervision, and professional development provided

What You’ll Do

This position combines community-based case management, Transitional Age Services for Young People, and school/community-based prevention programming.

Community Case Management focused on Transitional Age Young People

Support transitional-age youth as they build the skills, confidence, and resources needed to successfully move toward adulthood and greater independence.

Through individualized, community-based support, you will:

• Help young people develop practical independent living skills, including budgeting, employment, education, housing, transportation, meal planning, healthy relationships, and self-advocacy.
• Identify individual strengths, needs, goals, and barriers to independence.
• Build skills in communication, problem-solving, decision-making, coping, and navigating everyday challenges.
• Connect youth with community resources, services, and natural supports.

• Provide engaging, hands-on opportunities for youth to practice skills they can use in real-life situations.

• Coordinate services with therapists, caregivers, schools, community providers, and other members of the youth's treatment or support team.

• Advocate for youth and families and helping them navigate community systems and services.

Transitional Age Services

Help adolescents and young adults build practical skills and confidence as they prepare for greater independence.

Through individual and group activities, you will help young people develop skills related to daily living, future planning and practical real-life scenarios.

Prevention Services

Provide structured prevention programming in schools and community settings to strengthen protective factors, build skills, and reduce risk factors before more significant concerns develop.

Responsibilities may include:

• Facilitating evidence-informed prevention curricula, groups, and activities with children, adolescents, and families.

• Teaching social-emotional, coping, communication, problem-solving, and healthy decision-making skills.

• Creating engaging and developmentally appropriate learning experiences for youth.

• Building collaborative relationships with school personnel and community partners.

• Supporting outreach, recruitment, and engagement for prevention programs and activities.
