Case Manager - RN -All Care Hospice Northeast PRN

We’re seeking the right candidate for the position of a PRN RN Field Case Manager – RN at All Care Hospice to serve Northeast Louisiana. In this vital role, you’ll ensure that our residents receive the highest level of care while collaborating with a dedicated team, providing exceptional clinical leadership and support.

What You Bring to the Table:

Essential Qualifications:

• · Identify the patient/family's physical, psychosocial, and environmental needs and reassess as needed
• · Provide nursing services in accordance with the POC.
• · Document problems, appropriate goals, interventions, and patient/family response to hospice care.
• · Collaborate with the patient/family attending physician and other members of the IDT in providing patient and family care.
• · Instruct patient/family in self-care techniques when appropriate.
• · Supervise ancillary personnel and delegates responsibilities when required.
• · Complete and submit accurate and relevant clinical notes regarding the patient's condition.
• · Perform supervisory visits to the patient's residence.

The Ideal Candidate:

• · Applicants should possess and current Louisiana RN license in good standing and have at least one year of nursing experience.
• · Home health experience is preferred, but we will train eager candidates with good clinical experience.
• · Driver’s license, insurance and vehicle
• · Utmost compassion and integrity

Other Qualifications

• · Has not been excluded from participating in Medicare and Medicaid programs.
• · Maintains eligibility to participate in Medicare and Medicaid programs throughout employment.
• · Capable of providing emergency care as needed for any patient.
• · Maintains a neat, well-groomed appearance at all times. Keeps personal appearance appropriate for patient care.
